Privacy Policy

E2E PGx is the trade name of End to End Pharmacogenomics, LLC, a Minnesota limited liability company. This policy explains what happens to information collected through e2epgx.com.

It covers this website only. Information we receive in the course of a consulting engagement is governed by the agreement for that engagement rather than by this policy.

Information you give us

The contact form is the only place on this site where we ask you for information. It collects your name, email address, organization, organization type, and whatever you choose to write in the message field. We use that information to respond to your inquiry and to scope potential work, and we do not sell it, rent it, or share it outside E2E PGx except with the service providers described below.

Please do not send patient information through this form. It is not a secure channel, and it is not intended for protected health information, patient identifiers, or clinical records. If an engagement requires us to receive data of that kind, we will put the appropriate agreement and a secure method of transfer in place first.

Information collected automatically

We use Cloudflare Web Analytics to see how many people visit the site and which pages they read. Cloudflare states that Web Analytics does not use client-side state such as cookies or localStorage, does not fingerprint visitors by IP address or user-agent string, and does not track individuals across sites. It reports aggregate page views rather than identified visitors.

The site is served through Cloudflare's network, which processes routine request data — including your IP address — in order to deliver pages and protect the site against abuse.

Cookies

This site does not set cookies, and it carries no advertising and no cross-site tracking. That is why you are not being asked to accept anything.

Service providers

Cognito Forms hosts the contact form. Submissions are stored on Cognito Forms' systems in United States datacenters operated by Microsoft Azure. Cognito Forms acts as our processor and states that it does not use cookies on embedded forms and does not track the people who fill them out. See the Cognito Forms privacy policy.
Cloudflare provides hosting, content delivery, and the analytics described above. See the Cloudflare privacy policy.
Google Fonts serves the two typefaces used on this site. Loading a font sends your IP address and browser user-agent to Google. A browser setting or extension that blocks third-party font loading will prevent that request without affecting anything else on the site.

How long we keep it

We keep contact-form submissions for as long as we reasonably need them for your inquiry and any engagement that follows from it, and then we delete them. Cognito Forms states that deleted data is purged from its backups within thirty days.

Your choices

Write to bronwyn@e2epgx.com and we will tell you what we hold about you, correct it, or delete it. If you are in a jurisdiction that grants you a formal right of access or erasure, this is how you exercise it. We will not charge you for the request or ask you to justify it.

Children

This site is addressed to professional and institutional audiences, and we do not knowingly collect information from anyone under 18.

Changes to this policy

If we change this policy we will update the effective date above, and we will describe any material change in a short note at the top of this page.
